Documents to Submit

    • 6th Grade Report Card
    • 7th Grade Report Card
    • 8th Grade Report Card (Semester 1)
    • Standardized Test Scores from 6th and 7th-grade (if you have 8th-grade scores, please include those as well)
    • IEP/504 Documents (if applicable)
    • 6th, 7th Grade, and 8th Grade Attendance Records
    • 6th, 7th, and 8th Grade Grade Disciplinary Records
This is a requirement for admittance into CCHS and will include STAR Math and Reading, Otis-Lennon, and English Essay Prompt. We can accommodate up to 25 students per testing date and we will provide all testing materials and laptops.
